Position: Social Worker

Department: Social Work & Patient Flow - Outpatient Diabetes Program

Status: Temporary Part-time, 0.2 FTE (approximately one year)

Hours: Primarily Days

Site: Multi-site (Oakville, Milton, Georgetown, Burlington)

Responsibilities:

As part of an outpatient treatment team, the incumbent will provide clinical social work services to outpatients of the Halton Diabetes Program. Conduct social work and psychosocial assessments on outpatients referred by the program, using a variety of modalities and in accordance with professional standards, clinical policies and practices. The incumbent will provide selected therapeutic interventions to patients and families including individual and family therapy, supportive counseling and education, community outreach and advocacy.They will promote high standards of ethical clinical care and will lead the 12 month follow-up group and group sessions on Stress Management and Diabetes.

Qualifications:

Halton Healthcare is seeking individuals who are competent in their chosen profession; patient/customer focused; team players; effective communicators; caring and respectful and are fiscally responsible.

  • Member of the OCSWSSW
  • BSW or MSW
  • Experience providing scope of practice social work to outpatients with diabetes. 
  • Brief Solution focused therapy experience preferred.
  • Strong knowledge of community resources and demonstrated ability to work collaboratively and effectively with community partners.
  • Demonstrated ability to maintain an evidence-based, best practice approach.
  • Demonstrated excellence in interview skills.
  • Experience running groups for patients with chronic illness.
  • Experience working within and knowledge of philosophy of Self Management Model of Care
  • Satisfactory performance and attendance record.

Salary: $32.81 to $41.80 per hour
