The Department of Family and Community Medicine (DFCM) is strongly committed to the care of the inner city population. In addition to providing exemplary patient care and academic teaching in all aspects of primary care medicine, our staff has developed expertise in a number of areas in response to the needs of the community we serve. These areas include HIV, primary care, addiction medicine, and the care of people who are homeless or under-housed, the poor as well as adolescents and new immigrants. Clinical activities and targeted patient programs in these areas are fully integrated within the day-to-day practice of family medicine.

The Community and Patient Engagement Specialist will work alongside the St. Michael's Hospital Academic Family Health Team (SMHAFHT) management team and report directly to the Senior Clinical Program Director.

  • Provide expertise and leadership in the area of health equity
  • In collaboration with the QI Specialist support quality improvement initiatives that reflect feedback received from patients
  • Develop and build relationships with internal and external stakeholders and patients;
  • Provide administrative support as the key liaison to the AFHT Patient Advisory Council
  • Develop patient communication and engagement strategies in consultation with the Patient Advisory Council
  • Provide oversight for the patient comfort fund and closet
  • Advocate on behalf of patients of the SMHAFHT at the senior level to ensure representation of the patients voice in strategic planning and other departmental activities
  • Develop strategic objectives for health equity and community and patient engagement in a manner that builds upon the SMHAFHT vision


  • University Degree or Diploma in Health / Social Services or related field with a minimum of 5 years of related work experience
  • Proven project/program management, interpersonal and administrative skills, including the ability to set priorities, establish timeframes, and organize resources
  • Strong strategic, analytical, problem-solving, facilitation and negotiation skills, as well as systems thinking ability
  • Experience in developing and supporting collaborative relationships with patients
  • Excellent communication (verbal and written) skills, particularly with respect to the interpretation and presentation of evidence and in the mobilization of project teams;
  • Experience interacting with individuals from diverse ethnic and socio-economic backgrounds
