Knowledge, Skills, and Position Summary: The Outreach Case Manager is responsible for completing 1-2 days of outreach with the regional outreach and engagement team in places that are not meant for human habitation which include under bridges, encampments, parks, highways, etc in addition to case management duties. This case manager is actively reaching out and working with clients who are literally homeless and connecting them to mainstream services including the Greater New Haven CAN, 211, mental health services, recovery services, and, most importantly, housing services. Responsibilities include delivering comprehensive case management services to clients including intake assessments, the development and implementation of individual care plans, being a liaison with treatment providers and referrals to community resources. This Case Manager should have strong organization skills and documentation skills. The tentative schedule for this position is 32 hours per week, Monday thru Friday, 9am -3:15pm with flexibility to work 7am-12:15pm or 12:45pm-7pm, once or twice a week regularly. This position is non-exempt and works under the supervision of program management. Regular and predictable attendance is required for this position.

Education: The minimum qualifications to be an eligible candidate for the Case Manager 2 position is an Associate’s degree and 2 years of related experience in the direct service and case management field. Valid Connecticut driver’s license is required for all open positions at The Connection.

Orientation: If selected for employment with The Connection, newly hired employees will be required to attend New Hire Orientation on days 1-3. This orientation is held at our Main Office in Middletown, Connecticut.
