Location: Day Program @ Our Lady of Fatima
Primary Function: To develop, schedule, implement, monitor and train staff of services according to each individual’s program plan within his/her caseload.
Job Responsibilities:
- Effectively maintain a maximum caseload of 30-day program participants.
- Responsible for the coordination and completion of initial day program assessments for each participant.
- Responsible for participating and attending in the development of ISP/IPP.
- Ensure that required information is maintained and implemented as referenced in the plan.
- Complete monthly documentation on the participant’s participation and progress.
- Serve as a point of contact for families and agency representatives.
- Other duties assigned.
Requirements:
- Bachelor’s Degree; Masters preferred
- 2-4 years’ experience working with individuals with intellectual disabilities
- Must have a valid Driver’s License with 3 years licensed driving experience.
